Class Pass Request Form | Fun Trips


Please remember the following:

    • Submit the request at least two (2) weeks prior to the trip.
    • Make sure to fill in ALL the requested information.
    • Trips must be taken between 9:00 AM and 2:00 PM.
    • Class Pass valid only on issue date.
    • Keep group number at up to 30 people, including chaperones.

If having a problem submitting form online, please use Downloadable Request Form from previous page and contact us at valvarez@lavta.org to explain the issue
